Use this form to request a summary of your reports to date. An email will be sent to you with a list of your Activity and Catch Reports. The summary will also indicate additional reports that may be required but not yet received by the DFO.

Important Notes

When you click the "Send Email" button, the data on this form will be copied into a new email message that is addressed to Archipelago Marine Research (AMR). You may have to click the Send button in your email program in order to complete the process. The AMR email system will forward the request to a DFO computer system that will automatically check it for accuracy and then send you a reply email. If you send your request between the hours of 05:00 and 21:30, you should receive a reply within 30 minutes. If you send a message after 21:30 you may not receive a reply until after 05:00 the following morning.

In order to use this form, you must first meet the following conditions:

• your email address and vessel master FIN are registered with your logbook number at AMR (call 1-877-280-3474 to do this);
• you are on a computer or mobile device with an email client program (e.g. Outlook, Mac Mail) that is configured for an email address that is registered with your logbook.
